Time to file claims. Unless a longer timeframe is required under applicable law, all information necessary to process a claim must be received by United no more than 90 days from the date of discharge or from the date outpatient Covered Services are rendered. If Payer is not the primary payer, and Facility is pursuing payment from the primary payer, the timely filing limit will begin on the date Facility receives the claim response from the primary payer. In the event United requests additional information in order to process a claim, Facility will provide that additional information within 90 days of United’s request, unless a longer timeframe is required under applicable law.
