Transitional Case Management. The Contractor shall implement procedures to coordinate the services that it furnishes to the enrollee between settings of care, including appropriate discharge planning for short-term and long-term hospital and institutional stays as required by 42 C.F.R. §438.208(b)(2)(i). The Contractor shall provide transitional case management for enrollees to support transitions between institutional and community care settings, including but not limited to, transitions to/from inpatient hospitals, nursing facilities, psychiatric facilities, PRTFs, therapeutic group homes, permanent supportive housing, intermediate care facilities, residential substance use disorder settings, and transitions out of incarceration. Transitional Case Management. The evaluation of an enrollee’s medical care needs and coordination of any other support services in order to arrange for safe and appropriate care after discharge from one level of care to another level of care, including referral to appropriate services.
