Voluntary Acceptance. Employees are under no legal obligation to the District or to a college/university involved to accept a student teacher, intern, or practicum student. If the employee does accept this responsibility, he/she does so on a voluntary basis.
Voluntary Acceptance. The acceptance of the health-medical-dental insurance program is voluntary on the part of eligible employees; however, no additional compensation will be provided to those employees who are eligible and choose not to participate.
Voluntary Acceptance. A. Employee states clearly and without any reservation that their entering into this Agreement is done voluntarily and for the purpose of taking advantage of this program offered by the University.
B. Employee further acknowledges that no person, organization, employee, officer or agent of the employer has suggested or otherwise attempted to cause, force or coerce the Employee to involuntarily take advantage of this program and to terminate employment with the University.
Voluntary Acceptance. Executive hereby declares Executive has read this Agreement and that, at all times in the negotiation, review and acceptance of this Agreement and Executive has had adequate time to discuss this Agreement with Executive’s personal attorney, if Executive chooses to do so. Executive fully understands and appreciates the meaning of this Agreement and Executive has executed the same of Executive’s own free will and accord, which was not procured through fraud, duress or mistake and has not had the effect of misleading, misinforming or failing to inform Executive in any way.
Voluntary Acceptance. By signing this Agreement and Release, you acknowledge and agree that:
a. You have entered into this Agreement and Release knowingly and voluntarily;
b. You have read and understand, the entire Agreement and Release and Addendum A;
c. You have been advised in writing to consult with counsel of your choice before signing this Agreement and Release and Addendum A;
d. You have been provided with information in writing (see Attachment A) about:
(i) The class, unit or group covered by the offer of an opportunity to enter into an Agreement and Release in exchange for benefits;
(ii) The eligibility factors for the offer of an opportunity to enter into an Agreement and Release in exchange for benefits;
(iii) Any time limits applicable to the offer of an opportunity to enter into an Agreement and Release in exchange for benefits;
(iv) The job titles and ages of all individuals who are eligible for the opportunity to enter into an Agreement and Release in exchange for benefits;
(v) The job titles and ages of all individuals in the same job classification or organizational unit who are not eligible for the opportunity to enter into an Agreement and Release in exchange for benefits.
Voluntary Acceptance. A bargaining unit member may decline a student/teacher placement at the time it is offered.
Voluntary Acceptance. The parties hereby declare that the terms of this Release Agreement have been completely read and are fully understood and are voluntarily accepted for the purpose of making a full and final compromise, adjustment and settlement of any and all claims, disputed or otherwise, on account of the alleged injuries and damages above mentioned, and for the express purpose of precluding forever any further or additional claims of the parties arising out of their relationships prior to the date of the execution of this Release Agreement.
Voluntary Acceptance. EXECUTIVE STATES THAT EXECUTIVE HAS READ AND UNDERSTANDS THIS RELEASE AND THAT EXECUTIVE IS EXECUTING THIS RELEASE KNOWINGLY, VOLUNTARILY, WITHOUT COERCION AND AS EXECUTIVE’S OWN FREE ACT AND DEED. EXECUTIVE IS ADVISED TO SEEK THE ADVICE OF COUNSEL IN CONNECTION WITH THE EXECUTION OF THIS RELEASE.
Voluntary Acceptance. I understand and agree that I have entered into this Agreement voluntarily and as a statutory requirement for receipt of the scholarship.
Voluntary Acceptance. The Parties warrant and represent that they have fully read this Agreement, that they have fully discussed all the terms, conditions, and consequences of this Agreement with attorneys of their own choice, that they have freely and without reservation relied upon the advice of their attorneys in entering this Agreement, and that they therefore completely understand and voluntarily accept all the terms, conditions, and consequences of this Agreement.
