W-9 and Tax Documentation. Within fourteen days of the Effective Date of this SCIA, the Contractor shall provide the UCRC a fully executed Internal Revenue Service Form W-9, including a Taxpayer Identification Number or Employer Identification Number, as applicable. A blank W-9 form is provided to Contractor at the time of execution of this SCIA.
