Xxxxxxxxx Eligibility. Program staff will make every effort to determine eligibility within two (2) weeks of referral based on client’s availability. If client is unavailable or unable to schedule an appointment within the two-week period, Contractor must document all efforts/communication to schedule the appointment. Eligibility documentation must be kept in client (family’s) file.
