YOUR DIRECT PAYMENT. If you are an adult your eligibility for social care support was agreed using the Government’s eligibility criteria for Adult Social Care. You have completed a support plan and agreed that the support described in the plan is to be provided wholly or partly through a direct payment. If the direct payment is for a child, the social worker will have agreed with the parent the amount of and type of support that is to be provided through a direct payment. London Borough of ▇▇▇▇▇▇ will ensure that your direct payment, including your own financial contribution, is sufficient to meet your eligible needs as identified in your assessment of need and shown in your support plan. Once your support plan is agreed, you will receive written confirmation of the amount of your direct payment and the date that you will receive the first payment.
