I, , owner of real property(s) located at , Lewiston, Idaho, which real property is provided with City Utilities under Account # (s) understand and agree that, in the event utility charges under this account become delinquent as referred to in Lewiston City Code Section 36.5-19, I will remain responsible for payment of this account as provided in Lewiston City Code Section 36.5-16 including reconnection fees and late fees. Further, I voluntarily authorize a tax assessment to this property if the account remains unpaid as provided in Lewiston City Code Section 36.5-23 and Lewiston City Code Section 36.5-24. This is not an all-inclusive list of applicable sections of the Code, additional sections may apply; please refer to the Lewiston City Code.
