(“Agreement”) is executed by “Releasor” or “you” (as defined below) in favor of Boost Child & Family Services, Inc., a Connecticut corporation with a place of business located at 19 Compo Road South, Westport, Connecticut 06880, together with its members, officers, employees, contractors, representatives, subsidiaries and other affiliates (collectively, “Boost & Affiliates”). The Releasor and Boost & Affiliates are collectively referred to as the “Parties.”
