The undersigned employee (or, if unsigned, the employee who receives this agreement and reports to work and/or continues his or her employment after the Effective Date) (“Employee”) and Culinary Services of America, Inc. (dba Culinary Staffing Service), on behalf of itself and all of its parents, affiliates and subsidiaries (collectively, the “Company”), hereby enter into this Mutual Agreement to Arbitrate (“Agreement”). Employee and Company together shall be referred to as the “Parties.”
