I, [name of participant], (hereafter “the Participant”), grant to the Student Producer, and the Student Producer’s crew and other agents (collectively, the “Producer”), the absolute and irrevocable right and permission to use my name, voice, signature, photograph, image, likeness, and/or distinctive appearance (collectively, “my Likeness”) in any medium (whether now known or hereafter created) and in perpetuity for the purpose of creating a film currently titled (the “Film”) and to promote and advertise the Film (Advertising Materials”).
