This Permission Request Form/Agreement must be completed and signed on page 2 by the applicant and submitted to the DPIL Regional Director for review by Penguin Random House LLC for each separate one-time use requested. The request must be received at least 60 days in advance of intended use. The Permission Agreement (including the permission grant) on page 2 is not effective until signed by Penguin Random House LLC.
