The parties to this HIPAA Addendum (“Addendum”) are Michigan Health Information Network Shared Services (“HIN”) and (“Participating Organization” or “PO”). This Addendum supplements and is made a part of the Simple Data Sharing Organization Agreement between the Parties (“Agreement”).
