The Practice delivers primary care medicine to patients in a direct pay primary care, membership model (DPC), at 212 Carter Drive. Suite C, Middletown, Delaware 19709. In exchange for certain periodic fees, the Practice agrees to provide the Patient/Member with the primary care services described in this Agreement, under the terms and conditions contained within.
