As a custodial parent or legal guardian, I give full permission for the below-named Child to participate in supervised and unsupervised, voluntary athletic activity – including workouts, training sessions, and practices – between June 15 and August 12, 2020. I acknowledge that I have had the opportunity to ask questions and obtain whatever information I require to fully inform myself about the activity as well as the risks that my Child may be exposed.
