IN CONSIDERATION for being permitted to attend the Charter Township of Independence Parks and Recreation Day Camp (the "camp"), including, but not limited to, participation in camp activities and use of recreational facilities or equipment, the undersigned, on behalf of himself or herself and such participating children and any personal representatives, heirs, and next of kin (hereinafter referred to as "the undersigned") hereby acknowledges, agrees and represents that he or she has carefully considered the activities to be conducted and the equipment and facilities to be used and that the undersigned finds and accepts same as being safe and reasonably suited for participation by such participating children.
