This Client Agreement (the “Agreement”) is entered into on this day of , 2021 (“Effective Date”) by and between North Penn School District, located at 401 East Hancock Street, Lansdale, PA 19446 (“Client”), and Everside Health, LLC, located at 1400 Wewatta Street, Suite, 350, Denver, CO 80202 (“Everside”) and Paladina Health Medical Group, PC (“Everside PC”). Client, Everside, and Everside PC are each individually a “Party” and collectively the “Parties” to this Agreement.
