(Name of Company, hereinafter called the “Corporation”) wishes to establish a Corporate Membership with the CHRISTUS Good Shepherd Health System Institute for Healthy Living. Said Corporation recognizes the use of its company titles and agrees to maintain the five (5) adult minimum for the corporate discount on membership.
