Together, this Transmittal Form and the attached License Agreement constitute the Online Courseware Intellectual Property Agreement (“IP Agreement”), in compliance with UNT Policy 06.032, Online Courseware Intellectual Property. Faculty Member (“Author”) consultation with the Department Chair is advised. The Director of the UNT Center for Learning Enhancement, Assessment, and Redesign (“CLEAR”) is available to answer procedural questions; while legal questions should be referred to UNT System Office of General Counsel. Please acquire signatures in the order indicated on this Transmittal Form to ensure efficient processing of the IP Agreement.
