The Charter School’s Governing Board Chair hereby authorizes M-DCPS to initiate credit entries to the referenced account in the financial institution listed below and authorizes the financial institution to credit the same to this account. This authority is to remain in effect until revoked by the school’s Governing Board Chair in writing to M-DCPS. Account changes should be reported to M-DCPS thirty (30) days prior to the actual change. Please complete the following information which includes requiring the financial institution to confirm and return the original signed form to M-DCPS:
