This MDU Access Agreement (the “Agreement”) is effective as of the Effective Date, and is made between Google Fiber Inc. and its subsidiaries (“Google Fiber”) having offices at 1600 Amphitheatre Parkway, Mountain View, California 94043 and Sheridan Berthiaume, having offices at the address set forth on the signature block below (“Owner”).
