I, acknowledge that I have read the terms of service agreement and have retained the services of Tennessee Valley Coatings, LLC. I understand that the services provided by Tennessee Valley Coatings, LLC may affect the reliability and operation of some part(s). I am aware that it is my responsibility to have the par(s) inspected by a qualified individual and have them deemed fit for service prior to placing the part(s) back into service. I am aware of any break in procedures and cleaning processes that are required for proper appearance and performance of part(s).
