THIS CDBG AGREEMENT (hereinafter the “Agreement”) is made and entered into by and between the City of Orlando, a Florida municipal corporation, with a principal address of 400 South Orange Avenue, Orlando, Florida, 32801 (hereinafter referred to as the "City"), and GMH Property Holdings, Inc. (“GMH”), a Florida not-for-profit corporation, with a mailing address of 1417 E. Concord St., Orlando, FL 32803 (hereinafter referred to as “GMH” or “Subrecipient”) and Grace Medical Home, Inc., a Florida not- for- profit corporation, with a mailing address of 1417 E. Concord St., Orlando, FL 32803 (hereinafter referred to as “Grace”).
