This Administrative Services Agreement (this “Agreement”) is made effective this [[•]] (the “Effective Date”), by and between Southern Pilot Insurance Company, a Wisconsin stock insurance company (“Southern Pilot”), and Forza Insurance Services, LLC, a Delaware limited liability company (“FIS”).
