ADULT PARTICIPATION AGREEMENTAdult Participation Agreement • September 29th, 2015
One form must be completed for each person attending who is out of high school and 18 or older. Chaperones must be 21 or older. This completed and signed form should be returned to your parish/school group leader. Group leaders will submit this paperwork to the LiFE Office along with payment. Checks are to be made payable to the “Life & Family Evangelization Office” or “LiFE Office”.
