The undersigned, , a(n) (“Recipient”), on and as of this day of , 2024 (“Effective Date”), hereby agrees, for the benefit of one or more entities affiliated with TRANSFORM OPERATING STORES LLC, a Delaware limited liability company (collectively, “Transform”), to all of the terms and conditions contained in this CONFIDENTIALITY AGREEMENT (this “Agreement”). Transform and Recipient may be collectively referred to in this Agreement as the “Parties” and each individually referred to as a “Party.”
