This Forbearance Master Services Agreement ("Agreement") is between the CenturyLink Incumbent Local Exchange Carrier (“ILEC”) entities listed in Attachment 1 of this Agreement (collectively and individually referred to as (“CenturyLink”) and the Consolidated Communications Enterprises Services, Inc. Competitive Local Exchange Carrier (“CLEC”) entities listed in Attachment 2 of this Agreement (collectively and individually referred to as (“Customer”) and becomes effective on the date the first Service Schedule is added (the “Effective Date”). This Agreement provides the terms and conditions applicable to Customer’s purchase of the specific products and services delineated herein (“Service”) from CenturyLink.
