This Oregon Health Authority 2015-2017 Intergovernmental Agreement for the provision of Environmental Health Services (the “Agreement”) is between the State of Oregon acting by and through its Oregon Health Authority (“OHA”) and Yamhill County Public Health, the entity designated, pursuant to ORS 431.375(2), as the Local Public Health Authority (“LPHA”).
