This Professional Services Agreement (“Agreement”) is made and entered into by and between the Cal Poly Corporation (“Corporation”), a nonprofit 501(c)(3) public benefit corporation and auxiliary that provides support to California Polytechnic State University (“University”), and [SERVICE ROVIDER]., [ENTITY TYPE], (“Service Provider”), each referred to as a “Party” or collectively the “Parties”. This Agreement shall be effective on the date that the last authorized signature is affixed (“Effective Date”).
