In consideration of participating in the COLLEGE OF ALAMEDA YOUTH SPORTS CAMP/CLINIC/GAME, I represent that I understand the nature of this sport and that my son/daughter is in good health, and in proper physical condition to participate in such sport. I acknowledge that if I believe the youth sports camp/clinic/game conditions are unsafe, I will immediately discontinue my son/daughter’s participation in the youth sports camp/clinic/game.
