This agreement is entered into by and between Prairie St. John’s (hereinafter referred to as PSJ) and West Fargo Public Schools (hereinafter referred to as WF Schools). Both parties hereby agree to the following terms and conditions for the primary purpose of providing outpatient treatment, both Partial Hospitalization and Intensive Outpatient Treatment to students whose insurance companies deem the service not medically necessary, however WF Schools requests continued services.
