This agreement, when signed by customer and approved by JSD, is a contract under which 1) JSD agrees to furnish to the Service Property Address water and/or sewer service(s) as shown in the “Service and Customer Account Information” set forth below as completed by JSD, 2) Customer agrees to pay for such service(s) in accordance with the rates, pricing requirements, rules and regulations as adopted from time to time by JSD and 3) Customer agrees to abide by the terms of the JSD Customer Policies as adopted from time to time by JSD.
