The CareVu Submitter Enrollment Form must be completed and returned in order to enroll a new provider/submitter. Upon receipt of this document, the new submitter will be configured in the CareVu Clearinghouse system. The submitter will receive a confirmation email or a phone call to indicate the enrollment with CareVu has been completed.
