CLINICAL TRIAL AGREEMENT This CLINICAL TRIAL AGREEMENT (the“Agreement”) is effective on 30 June 2020 (the “Effective Date”), by and between Nemocnice Jihlava p.o. located at Vrchlického 59, 586 33 Jihlava, Czech Republic, IČO (Company ID): 00090638, DIČ: CZ00090638, represented by MUDr. Lukáš Velev, MHA, director (the “Institution”), H.
