This Agreement for Disclosure and Transfer of Confidential Information and Data (“DUA” or “Agreement”) is effective as of the date of the last signature below (“Effective Date”) by and between the Chapel Hill Carrboro City Schools Board of Education, located in Chapel Hill, North Carolina (“SCHOOL SYSTEM”), and Duke University, a tax-exempt research and educational institution located in Durham, North Carolina, acting for and on behalf of its Duke Clinical Research Institute (“RECIPIENT”).
