This Participant Agreement made this day of January, 2020 between the Evergreen Animal Protective League, a 501(c)3 designated organization under the laws of Colorado with an address of PO Box 2517, Evergreen, Colorado 80437 (“EAPL”) and , residing at , as the registered owner of a canine nominee named for the Evergreen Honorary Mayor Program (“Dog Owner”), (together the “Parties”) sets forth the understanding and obligations of the Parties as to EAPL’s Honorary Mayor Program.
