This document sets forth the terms pursuant to which I, , have agreed voluntarily to donate some of the eggs (called "oocytes") which I will produce during my menstrual cycle to ("Recipient"), who has been unable to have children and who wishes to attempt to have children through the treatment process referred to as in vitro fertilization ("IVF"). The egg donation process will be undertaken through the Vincent Reproductive Medicine & IVF Program, Massachusetts General Hospital. Said program, including respective employees, agents and assigns shall be referred to in this document as the "IVF Program".
