THIS AGREEMENT is made and entered into this day of , 20 , by and between the City of Thornton, a Colorado home rule municipality, in the State of Colorado (hereinafter, “Thornton”) and _(the “Service Provider”). Thornton and Service Provider hereafter may be referred to collectively as, the “Parties” or individually as the “Party.”
