Prospective relative guardian(s) have the right to consult with an attorney before signing this agreement. Prospective relative guardian(s) must sign and receive a copy of this agreement. This agreement must be submitted and receive final approval by the social services district named below before the letters of guardianship are issued by the court to the prospective relative guardian(s) on behalf of the child named in this agreement.
