The young person listed below wants to take part in PHASTAR’s thematic programs, including water-based experiences on boats. These programs are conducted by United States Coast Guard (“USCG”) certified captains (“Master”). These captains operate PHASTAR-owned vessels that are also licensed by the USCB. I am the young person’s parent or legal guardian and I represent and warrant that I have the authority to enter into this agreement. In exchange for being allowed to take part in the PHASTAR Marineprograms, the young person and I agree as follows:
