(please print), I have given permission for him/her to participate in the Federal Way Teen Safety Academy on June 24-25, 2019, sponsored by the Federal Way Police Department and South King Fire & Rescue. I recognize that every effort will be made to keep my child safe, but there is some inherent risk of injury in some of the activities. Therefore, I agree to hold harmless and indemnify the City of Federal Way and South King Fire & Rescue from any and all damages incurred by my minor child, or dependent, named above, as a result of bodily injuries sustained by such minor in connection with the Academy at the local government facilities.
