I, the undersigned, hereby request that the identified Santa Clara County Federal Credit Union (SCCFCU) sponsor/employer group, association or organization (“Organization”) be granted permission to use SCCFCU’s Member Center Community & Conference Rooms (Meeting Facilities). I confirm that I am authorized to submit this request and enter into this Agreement on behalf of the identified organization. I agree that use of the SCCFCU Meeting Facilities by my Organization and its guests, if any, will be subject to compliance with this Agreement, with all applicable SCCFCU rules, and with applicable state, local and federal laws.
