The Health and Human Services Commission (“HHSC” or “System Agency”) and Coastal Plains Community MHMR Center (“Grantee” or “Contractor”), who are collectively referred to herein as the “Parties,” to that certain Coordinated Specialty Care (“CSC”) contract effective March 19, 2019 and denominated HHSC Contract No. HHS000349100001 (the “Contract”), now now desire to amend the Contract.
