AGREEMENT FOR COVID-19 TESTING SERVICES (“Amendment”) is made and entered into this 23rd day of August, 2021 (“Effective Date”) by and between the Santa Monica-Malibu Unified School District of Los Angeles County, a public school district organized and existing under the laws of the State of California (“District” or “SMMUSD”), and DoctorNow, a California corporation with a business address of 9335 Reseda Boulevard, Suite 100, Northridge, California 91324-2968 (“Contractor”). District and Contractor may be individually referred to herein this Amendment as a “Party” or may be collectively referred to as the “Parties.”
