WAIVER AND RELEASE OF LIABILITY READ BEFORE SIGNING Please read this Agreement carefully and understand its content. Please be aware that this form can be viewed and submitted online or can be filled out at the front desk of MWC Sports Complex. The Agreement is valid for one (1) year after signing. After one (1) year, participants must execute and submit a new Agreement and update your profile with MWC Sports Complex.
