This addendum (this “Addendum”) is made as of the same date as the date of acceptance of the Quotation (as defined hereinbelow), between RaySearch Laboratories AB (publ.) a company existing under the laws of Sweden (“RaySearch”), and the [NAME OF CLINIC], (“Clinic”), each referred to herein individually as a “Party” and collectively as the “Parties”.
