Black Belt Examination Agreement Applicant:Examination Agreement • April 5th, 2012
The Educational Martial Arts System, Inc. (Here by known as TEMAS) hereby agrees to furnish a competent judge and adequate facility for the promotion examination. Upon passing the examination, TEMAS will issue a membership identification card and rank certification certificate in agreement with the Chief Instructor’s studio. I understand and agree that the promotion examination fee is non refundable under any circumstance.
