This Security Flex 125 Program Flexible Spending Account Service Agreement (the “Agreement”) is hereby entered into effective as of ______________________, 20__, between _____________________________________________________________ (the "Employer"), the Plan Sponsor of a cafeteria plan (the "Plan") governed by, and intended to comply with, section 125 of the Internal Revenue Code ("Code") and Security Benefit Life Insurance Company ("Security Benefit"), a provider of non-discretionary administrative services to sponsors of such cafeteria plans.
