This Agreement to be the exclusive provider of Advanced Life Support (ALS) and Basic Life Support (BLS) emergency/non-emergency ambulance service is made and entered into on this the day of , 2016 to be effective on the 10th day of May, 2016 (“Effective Date”) by Pafford Emergency Medical Services, Inc. (“Pafford”) and the County of Madison, Mississippi (“County”), upon the following terms and conditions.
