The undersigned enters into this Agreement with ClickMD Marketing, Dr. Robert Marvin, Houston Surgical Specialists, LLC and Nobilis Health (“Producers”). I have been informed and understand that the Producers are producing video/photgraphy and that my name, likeness, image, voice, appearance and/or performance are being recorded and made a part of that production.
