This is an Agreement between you, the Member, and Root Functional Medicine, PLC, “Root” located in Grand Rapids, MI. Root Functional Medicine is an insurance-free practice combining lifestyle guidance, health coaching, nutrition counseling, alternative medical lab orders for testing and review and physician consultations. In exchange for individual consultation fees or monthly subscription fees paid by you, the Member, Root Functional Medicine will provide the services described in this Agreement.
